The natural distance between the feet is directly related to the hip width.
The stance width can probably become larger because of more thigh circumference. But usually the anatomical
pelvic width also corresponds to the hip width.The shoe alignment sidewards orientates to this distance.

The position of the pedal axle is also depending on the shoe size

The distance from the metatarso-phalangeal-joint to the pedal axle varies.
The foot length is one variable.

Alignment

The personal feet alignment can be seen in the walking style. Externally or internally orientated heels generate different shapes of the footprints in the sand.

This characteristical footprint should be transfered to the pedals.
